
What is Health and Wellbeing Coaching?
At Oasis PCN, our Health and Wellbeing team primarily uses health coaching techniques to empower individuals with the knowledge, skills, and confidence to take an active role in managing their health. Our goal is to support people in achieving their personal health and wellbeing objectives.
We work closely with individuals to help them identify and manage existing conditions through tailored coaching sessions. These sessions focus on setting personal health goals, building motivation, and implementing a personalised health plan. Additionally, we provide access to self-management education, peer support, and social prescribing to further enhance overall wellbeing.
Every individual’s journey with a Health and Wellbeing Coach is unique, and sessions are customised to meet their specific needs. Typically, individuals participate in at least six sessions over a period of 4-6 months to ensure sustainable progress.
Health & Wellbeing Coaching is also available through co-produced webinars and group sessions, such as our Women’s Health Programme, which covers essential topics including menopause, health screenings, anxiety, mindfulness, healthy eating, exercise, and mental wellbeing resources like the ShinyMind App. These sessions are designed collaboratively to provide holistic support and help patients achieve realistic, agreed-upon goals.
